    The pronounced degenerative changes observed in gill indicate the vulnerability of tissue possibly due to its role as first contact and entry point for the pesticide. Consequently, clothianidin exerted its toxic effects by altering normal behavior, causing neurotoxicity and disturbing osmoregulation. Moreover, the imposed stress was responded in a tissue-specific manner and histological lesions become more severe with increasing concentration. The findings clearly reveal the potential threat caused by environmentally relevant concentrations of clothianidin to early life stages of fish.

    Originally developed for treatment of human immunodeficiency virus (HIV), the antiviral combination lopinavir/ritonavir (LPV/r) is being repurposed for treating the novel coronavirus disease (COVID-19) despite minimal experience in this markedly different population and an in-vitro derived EC50 against SARS-CoV-2 several hundred-fold greater than for HIV. We present a case series including a case of severe hyponatremia and a 32-fold overdose raising safety and effectiveness concerns in COVID-19 patients.

    We measured LPV trough concentrations in 12 patients and reviewed their clinical charts for side effects known to occur in HIV patients.

    Compared to established LPV trough concentrations in HIV patients, concentrations in COVID-19 patients were 3-fold greater (19.37 ± 10.12 mcg/mL versus 6.25 mcg/mL). In addition, cholestasis and dyslipidemia toxicity thresholds were exceeded in 12/12 and 11/12 patients respectively. No patients achieved the presumed therapeutic concentration. Side effects included gastrointestinal symptoms (5/12), electrolyte imbalances (4/12), liver enzyme disturbances (5/12) and triglyceride elevations (2/12).

The war in Syria has become notorious for targeted violence against health care. This qualitative study describes health care workers’ experiences of violence using semi-structured interviews (n = 25) with professionals who have been working in Syria. The participants were selected using a snowball sampling method and interviewed in Turkey and Europe between 2016-2017. Analysis was conducted using content analysis. Results revealed that the most destructive and horrific forms of violence health care workers have experienced were committed mostly by the Government of Syria and the Islamic State. Non-state armed groups and Kurdish Forces have also committed acts of violence against health care, though their scope and scale were considered to have a lower mortality. The nature of violence has evolved during the conflict starting from verbal threats and eventually leading to hospital bombings. Health care workers were not only providers of health care to injured demonstrators, they also participated in non- violent anti-government actions. The international community has not taken action to protect health care in Syria. For health workers finding safe environments in which to deliver health care has been impossible.Introduction Airway pressure in the first 100 ms of an occluded inspiration (P0.1) evaluates respiratory center activity. Its increase compensates for respiratory muscle weakness in amyotrophic lateral sclerosis (ALS). We aimed to evaluate if an exhaustion over disease progression emerges. Methods Consecutive ALS patients with P0.1 at baseline (T0) above 90th percentile for the whole population were included. There were two patient categories within this population G1 patients had respiratory function tests (RFT) at T0 and 6 months later (T1); G2 patients were immediately adapted to noninvasive ventilation at T0. Functional ALS rating scale (ALSFRS) and its subscores, RFT including maximal inspiratory pressure (MIP), P0.1 percentage (%P0.1), P0.1/MIP, blood gas analyses, phrenic nerve motor amplitude and nocturnal pulsed oximetry (NPO) were compared at T0 between groups (Mann-Whitney U test) and longitudinal progression in G1 was assessed (Wilcoxon test). p  less then  0.05 was taken as significant. Results Forty-nine patients were studied 13 men, mean disease duration 17.1 ± 19.5 months, 21 with spinal-onset; 27 bulbar-onset; one with respiratory presentation. In G1, there was a progressive deterioration in all variables but P0.1/MIP did not significantly change (p = 0.394). PCO2 (p = 0.045) and bicarbonate ion (HCO3-p = 0.054) increased. In G2, the ALSFRS (p = 0.049) scores were lower, PCO2 (p = 0.057) and HCO3- (p = 0.016) higher, and reduced arterial oxygen saturation (SpO2 less then 90%) at NPO (p = 0.007). Discussion Patients with high central respiratory drive responses did not develop a short-term “exhaustion phenomenon”. Both metabolic and central respiratory mechanisms interact to compensate for respiratory muscle weakness. The role of spasticity and the respiratory-metabolic-renal buffering system deserves further studies.In order to focus on the injury mechanism of electric cyclists in the collision accident between right turn of truck and electric bicycle, Pc-Crash is used to value the injury of the electric cyclist in this paper. Through repeated tests in Pc-Crash, it was found that two dynamic response process were existed. The electric bicycle and cyclist collided with truck, and the electric bicycle and cyclist were thrown to the ground due to the impact force. Finally, they were crushed under certain conditions. The multi-rigid-body truck model is built on the theories of multi-rigid-body modeling and multi-rigid-body collision to get the value of the crushed part which the single rigid model can’t calculate. After that, we analyze HIC, continuous 3 ms resultant acceleration of chest, the shear force of femur and leg, based on the electric bicycle speed, the mass of electric bicycle and cyclist, the height of cyclist’s seat. We found that when the electric cyclist is not crushed by the truck, the value of each damage index is below the threshold.
