    The probability to retrieve sperm with the MD-TESE technique is high in NOA cases where earlier sperm retrieval with blind methods such as needle aspiration, percutaneous needle biopsy, or conventional TESE has failed.Background Endovascular aortic aneurysm repair (EVAR) with stent-grafts is used widely for the treatment of thoracic aortic aneurysms (TAA). Inappropriate design of stent-grafts may lead to complications such as endoleak, stent-graft migration and new entries, causes of which may be inappropriate radial support force or insufficient longitudinal flexibility of the stent-grafts.Material and methods To improve the mechanical performance of the stent-grafts, a type of non-equal-strut stent hoops was proposed, and the influence of structural parameters on the mechanical performance was studied.Results Results of numerical simulation and physical experiments show that by using the proposed non-equal-strut stent hoops, radial support force and longitudinal flexibility of stent-grafts can be reconciled and balanced.Conclusion Results of this study could be used to facilitate radial force control and longitudinal flexibility enhancement in the design of aortic stent-grafts.Objective To study the mechanical properties of sciatic nerve in rats with chronic alcoholism (CA) and intervened with bone marrow mesenchymal stem cells (BMSCs) and to provide biomechanical basis for clinical practice.Methods the serum of the BMSCs-intervened CA rats was sampled and determined the contents of malondialdehyde (MDA), metallothionein (CAS, MT), and Glutathione/r -glutamyl cysteinyl/glycine (GSH); meanwhile, the rats’ sciatic nerve was tested the tensile and observed the histomorphological changes.Results The mechanical properties of sciatic nerve in BMSCs-intervened CA rats, as well as the serum levels of MT and GSH, were significantly different from those in the basic fibroblast growth factor (bFGF)-intervened CA rats (p  less then  0.05).Conclusions BMSCs intervention can restore the levels of MT, GSH, MDA, histomorphology, and tensile mechanical properties in CA animal model, and its effects on repairing sciatic nerve are obvious.Introduction Angiotensin receptor blockers (ARBs) are recommended as preferential drugs in the treatment of hypertension by guidelines. In this review, we reappraise their effectiveness to preventing major cardiovascular outcomes and the recent concerns with new adverse effects.Areas covered ARBs were not superior to placebo in the prevention of all-cause deaths and combined cardiovascular events in seven randomized controlled trials (RCT). Several meta-analyses, with large number of participants, confirmed these findings. Three RCT trials, published in top journals, were retracted due to the uncertain reliability of their findings. Beside the low effectiveness in the prevention of cardiovascular disease (CVD), ARBs were implicated in one meta-analysis with an increased risk of cancer a finding that was not confirmed in another meta-analysis. In observational studies, ARBs were associated with a higher incidence of suicides. In addition, several lots of ARBs were contaminated with carcinogens.Expert opinion ARBs have not been proven to reduce CVD risk and therefore should not be preferential drugs to treat hypertension. The implication of ARBs with suicide and cancer needs to be further studied.Background Optimal surgical management of monochorionic diamniotic twins complicated by selective intrauterine growth restriction (SIUGR) type II is unknown. Surgical management may involve selective laser photocoagulation of communicating vessels (SLPCV), which offers the possibility of dual twin survivors versus umbilical cord occlusion (UCO) of the SIUGR twin.Objective To identify patient characteristics associated with SIUGR twin survival for those undergoing SLPCV.Study design All patients studied were those who underwent fetal treatment for SIUGR type II at our center from 2006-2018. SIUGR type II was defined as an estimated fetal weight less then 10th percentile with persistent absent and/or reversed end diastolic flow in the umbilical artery of the SIUGR twin, in the absence of twin-twin transfusion syndrome. Patients were offered SLPCV versus UCO, and those undergoing SLPCV, patient characteristics associated with 30-day survival of the SIUGR twin were examined using bivariate analysis and multiple UGR type II patients by preoperative Doppler indices created two groups, one (type IIa) with 50% survival and one (type IIb) with 0% survival of the SIUGR twin after laser surgery. Upon further confirmation, these findings may provide guidance for counseling patients and conducting fetal therapy.Introduction Statins remain the most commonly prescribed lipid-lowering drug class for the treatment of atherosclerotic cardiovascular disease. Their well-recognized side effects are known as statin-associated muscle symptom (SAMS). Some advances in this field have been made in recent years, but the understanding of the mechanisms has lagged. Investigating the specific role of the anti-HMGCR autoantibody, pharmacokinetic genetic variants, characterization of the known phenotypes of statin toxicity, in relation to clinical markers of disease, is of high importance.Areas covered We summarized currently available findings (on PubMed) related to SAMS and discussed the therapeutic approaches, risk factors, drug interactions, potential novel systems, algorithms and biomarkers for SAMS detection. CoQ10 supplementation has been suggested as a complementary approach to manage SAMS, while vitamin D levels may be useful for both the diagnosis and management.Expert Opinion/Commentary Further studies might help to understand the easiest way to diagnose SAMS, suitable prevention and an effective non-statin therapy.
